How
to Check In
When
you arrive at our office at 3421 W. William Cannon,
Suite 143, whether you are a new patient or come on
a regular basis, you will notice a keyboard and monitor
in the window of our waiting room. Please type your
first and last name on the keyboard and
press "Enter." Don't worry about capitalization,
the computer doesn't care.
After
you enter your name, listen to the computer and it will
acknowledge your arrival. You will have to listen to
the computer's response and read the screen for further
instructions. You may be asked to see the receptionist,
or to sit down in the waiting area for a few minutes.
If
you have already completed one of our online forms or
you completed a form that was mailed to you, please
bring it with you to turn in to the receptionist. If
you have not completed a new patient form, the receptionist
will ask you to do so while you wait. If you need to
fill out paperwork in the office, please arrive 10-15
minutes early.
Preliminary
Examination (approximately 20-30 minutes)
The initial appointment is a visual examination
to determine your orthodontic needs. We will briefly
discuss the general nature of any pertinent problems
and advise of the proposed recommendations. This examination
is complimentary and no fee is charged. A more in-depth
analysis of your case will be done after the diagnostic
records are evaluated.
